About The Position

This role involves providing comprehensive social work services to clients. Requirements

Master's degree in Social Work (MSW) from an accredited program.
Strong communication, assessment, documentation, and interpersonal skills.
Knowledge of community resources and social service systems.
Ability to maintain confidentiality and handle sensitive situations professionally.

Nice To Haves

LMSW or LCSW preferred/required depending on the position.
Experience working with individuals, families, or vulnerable populations preferred.

Responsibilities

Conduct assessments to identify clients' social, emotional, and behavioral needs.
Develop and monitor individualized service/care plans.
Provide supportive counseling and crisis intervention as appropriate.
Coordinate referrals to healthcare, mental health, housing, financial assistance, and community resources.
Advocate for clients and help them navigate available services.
Maintain accurate and confidential client records and documentation.
Collaborate with healthcare providers, case managers, schools, families, and other professionals.
Participate in case conferences and multidisciplinary team meetings.
Monitor client progress and make recommendations for additional services.
Follow organizational policies and applicable federal, state, and local regulations.
