About The Position

The Social Worker provides emotional support, crisis intervention, and counseling to patients and families experiencing problems related to hospitalization, illness, disability, or trauma. The role includes providing information and referrals, including referrals for financial assistance and supportive services. The Social Worker provides case and milieu management, completes psychosocial assessments, assists with the development and reevaluation of treatment plans, and works as part of a multidisciplinary team. The role also coordinates discharge planning, completes required documentation, and maintains a clinical record for each client throughout their stay.

Requirements

Bachelor’s degree in Social Work, Marriage and Family Therapy, or another related field.
Current registration as an intern with the Board, as appropriate to education and licensing requirements.
Complete Behavior Violence Prevention training within three months of hire and maintain it annually.
Hospital-based case management or care coordination experience.
Strong discharge planning experience and the clinical knowledge to anticipate and address patient care needs.
Basic understanding of care transitions and alternate levels of care.

Nice To Haves

Minimum one year of inpatient or outpatient Behavioral Health experience.
Current BCLS (AHA) certificate upon hire and maintained as current.
Master’s degree in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Counselor, or a related field.
