About The Position

Luther Park Community is seeking a part-time Social Worker to support the Director of Social Services at the Trinity Center, a 120-bed skilled nursing facility specializing in Alzheimer's care. The role involves assisting residents in achieving and maintaining their highest practicable well-being, identifying medically-related social service needs, and ensuring these services comply with State and Federal regulations.

Requirements

  • A minimum of a bachelor’s degree in social work or another human services field (sociology, gerontology, special education, rehabilitation counseling, or psychology).
  • One year of supervised social work experience in a health care setting working directly with individuals.

Responsibilities

  • Assist in meeting and maintaining residents' highest practicable well-being.
  • Identify the need for medically-related social services and ensure provision in accordance with State and Federal regulations.
  • Complete or delegate the social services component of the comprehensive assessment.
  • Contribute to and/or direct/delegate social services goals and approaches to the comprehensive care plan, individualizing goals and interventions to match resident skills, abilities, interests, and preferences.
  • Promote individualized, non-pharmacological approaches to care that meet mental and psychosocial needs.
  • Facilitate residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
  • Assist residents and their representatives in locating and accessing financial, legal, and other community resources.
  • Coordinate implementation and oversight of procedures for documenting social services actions and interactions in resident medical records, upholding legal, ethical, and professional standards.
  • Assist the Social Services Director in overseeing the establishment and modification of departmental QA procedures.
  • Assist in the Advance Care Planning process for each resident upon admission and ensure regular review of Advance Directives.
  • Ensure staff are aware of resident code status and end-of-life wishes.
  • Inform and educate residents and their representatives about healthcare options and ramifications.
  • Participate in facility policy development to positively impact resident care quality.
  • Ensure staff are knowledgeable about Resident’s Rights and encourage staff to maintain and enhance resident dignity.
  • Advocate for residents and assist them in asserting their rights within the facility.
  • Report any allegations or suspected abuse, neglect, or exploitation to the Director/Administrator and appropriate State agency, and assist in investigations.
  • Ensure residents with mental illness or psychosocial difficulties have access to appropriate treatment and resources.
  • Work alongside other departments to ensure compliance with Trauma Informed Care.
  • Ensure all residents are assessed for trauma upon admission and regularly, and that trauma and interventions are reflected on the care plan.
  • Assist with the grievance process and educate staff, residents, and families about the grievance process.
  • Make arrangements for residents to obtain items such as clothing and personal care supplies.
  • Act as a resource for staff participating in behavioral interventions with residents.
  • Participate in Resident and/or Family Council as needed or requested.
  • Assist with or arrange for a resident’s communication of needs through their primary method of communication or in a language they understand.
  • Treat all residents with dignity and respect, and promote and protect all residents’ rights.
  • Establish a culture of compliance by adhering to all facility policies and procedures.
  • Comply with standards of business conduct and state/federal regulations and guidelines.
  • Maintain confidentiality of protected health information.
  • Report noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
  • Report any retaliation or discrimination to HR or the compliance officer.
  • Report any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.
  • Report work-related injuries and illnesses immediately to supervisor.
  • Contribute to facility efforts to maintain and/or improve quality of care through participation in Care Plan meetings, mandatory in-services, and facility-required training.
  • Successfully complete facility required training, and any Social Work continuing education and licensing requirements.
