About The Position

Four Seasons is a leading provider of hospice and palliative care, recognized for its commitment to both patient care and employee well-being. We are seeking a dedicated Social Worker to join our team in the South Region. This role involves providing essential psychosocial services to patients and their families, ensuring they receive comprehensive support aligned with our mission. The Social Worker will manage a caseload of hospice patients, offering services in various settings and collaborating with a multidisciplinary team.

Requirements

  • Bachelor's in Social Work (BSW) OR Master's in Social Work (MSW)
  • 1-3 years experience within a healthcare setting
  • Valid driver's license and proof of automobile insurance
  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
  • Knowledge of HIPAA privacy rules and regulations
  • Knowledge of electronic medical records systems
  • Expertise in discharge planning
  • Knowledge of Hospice Services to include Hospice Medicare Benefit
  • Demonstrate basic data entry skills

  • Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc.
  • Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies.
  • Perform duties in compliance with Medicare Condition of Participation.
  • Participate in the coordination of the patient’s hospice care in accordance with professional standards of practice.

  • Valid NC, Clinical Social Worker license (LCSW) preferred

Responsibilities

  • Perform initial and ongoing psychosocial assessments of patients, families, and caregivers, identifying support systems and barriers to care.
  • Implement and evaluate care plans for assigned patients and their families/caregivers.
  • Serve as a liaison to community health, welfare, and social agencies, making referrals as needed.
  • Provide patient and family counseling and education on family systems, dynamics, and coping methods.
  • Assist clients, families, and caregivers in determining healthcare and grief/loss goals.
  • Address advocacy needs for patients, families, and the organization.
  • Organize and facilitate patient discharge planning.
  • Actively participate in Interdisciplinary Team (IDT) meetings to develop and evaluate patient care plans.
  • Evaluate the financial needs of patients/families and make appropriate interventions for financial assistance.
  • Assist families/caregivers with final arrangements prior to or at the time of the patient's death.
  • Evaluate bereavement/pre-bereavement needs and develop a bereavement plan of care.
  • Assist patients and caregivers in understanding levels of care and their financial implications.
  • Obtain necessary information from third-party providers for payment.
  • Facilitate internal referrals.
  • Assist in providing 24-hour on-call psychosocial services.
  • Conduct pre-admission consults and/or facilitate completion of consent forms.
  • Attend staff and mandatory in-service meetings.
  • Participate in ongoing education and update professional skills.
  • Practice safety and infection control methods.
  • Perform other similar duties as assigned.
