About The Position

Reporting to the designated Social Work Site Leader, and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator, the Social Worker provides therapeutic clinical counselling and comprehensive biopsychosocial assessments. They provide consultative and direct services to patients/residents, families, staff and community health professionals, and offer information and support regarding community resources. The role involves providing leadership to the interdisciplinary team in the discharge planning process, coordinating clinical/case conferences, and maintaining patient/resident records. Additionally, the Social Worker develops and delivers in-services and presentations for staff, participates in committees and group meetings, and stays current with developments in the Social Work profession. The position is located at the new St. Paul’s Hospital and health campus, which offers an innovative approach to integrated care.

Requirements

• Master’s degree in Social Work from an accredited university, plus one (1) year of recent related experience in a designated program area, or an equivalent combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Demonstrated ability to communicate with and deal effectively with patients/residents, families, staff, physicians and other health care providers.
• Knowledge of legislation and government policies and procedures related to health care.
• Demonstrated ability to gather relevant data and complete biopsychosocial and patient/resident assessments.
• Knowledge of other health care disciplines and their role in patient/resident care.
• Demonstrated ability to draw out relevant information to understand the patient/resident’s situation and explore issues beyond the initial concerns.
• Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
• Demonstrated ability to deal with and/or guide others in conflict resolution.
• Ability to work independently, prioritize issues and workload, and facilitate group and inter-group meetings, processes and decision-making.
• Demonstrated ability to provide supervision and teaching to students and participate in research.
• Physical ability to carry out the duties of the position and ability to operate related equipment.

Responsibilities

• Provides therapeutic clinical counselling and comprehensive biopsychosocial assessments.

• Conducts comprehensive social work and psychosocial assessments by interviewing patients/residents and families, gathering relevant social information, and identifying factors that may impact coping and care needs.
• Maintains accurate patient/resident records, including assessments, progress notes, reports and other documentation in accordance with established standards, policies and regulatory requirements.
• Contributes to interdisciplinary discharge planning by assessing care needs, determining appropriate services and collaborating with hospital teams, community partners and other resources for patients/residents with complex needs.
• Provides direct and consultative support to patients/residents, families, staff and community health professionals regarding community resources, financial assistance and access to appropriate services.
• Participates in interdisciplinary care conferences related to patient/resident reviews, admissions, care planning and discharges.
• Supports patients, families and care teams in understanding relevant legislative processes, including substitute decision-making, Medical Assistance in Dying, Mental Health Act rights, incapability assessments and adult guardianship.
• Develops and delivers education, in-services, presentations and training for staff in collaboration with members of the interdisciplinary team.
• Facilitates and participates in group meetings, provides clinical counselling and delivers therapeutic and psychosocial education as required.
• Participates in program and practice evaluation, committees, research and quality improvement initiatives to enhance patient/resident care and service delivery.
• Maintains current knowledge of developments in Social Work practice and provides teaching, guidance and supervision to social work students as assigned.
