This position is responsible for completing psychosocial assessments for patients' post-hospital care and assisting patients in coping with stress related to their illness. The Social Worker collaborates with the Clinical Care Management team to assess and evaluate patients for appropriate care levels or settings and serves as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial components of a patient's illness. They provide information and education on community resources and options for post-hospital care, make referrals to community agencies, and report suspected cases of abuse, neglect, or exploitation. The role also involves serving as a liaison between the hospital, patients, and the Department of Social Services, and responding to requests for consultative services after normal working hours.
