About The Position

The Social Worker MSW position at Atrium Health Pineville Case Management is a part-time role focused on providing post-hospital care support to patients and their support systems. This role involves completing psychosocial assessments, assisting patients with coping mechanisms related to their illness, and collaborating with the Clinical Care Management team to determine appropriate care settings. The Social Worker will also serve as a resource for hospital staff and physicians regarding the emotional and social aspects of patient care, provide information on community resources, make referrals, and report suspected cases of abuse or neglect. Additionally, the position requires responding to after-hours consultative requests and taking on-call duties as scheduled.

Requirements

  • Masters in Social Work required
  • LCSW preferred
  • Applicable state Certification is preferred
  • Adherence to National Association of Social Workers Code of Ethics
  • At least 1 year professional experience in a hospital or health-related setting preferred
  • Expertise with Data Management Tools

Responsibilities

  • Completes psychosocial assessments for patients' post-hospital care and their support system.
  • Assists patients in coping with stress related to hospitalization, disability, and chronic/terminal illness.
  • Works with the Clinical Care Management team to assess and evaluate patients for appropriate levels of care or environment settings.
  • Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of patient illness.
  • Provides information and education to patients on community resources and post-hospital care options.
  • Makes referrals to community agencies as needed.
  • Reports suspected cases of child & adult abuse/neglect/exploitation.
  • Serves as a liaison between the hospital, patients, and the Department of Social Services.
  • Responds to requests for consultative services to patients after normal working hours.
  • Takes On-Call Duty as scheduled.
