This position is responsible for completing psychosocial assessments for patients' post-hospital care, assisting patients in coping with stress related to their illness, and collaborating with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting. The Social Worker serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness and its effects on their social support system. They provide information and education to patients on community resources and options for post-hospital care, make referrals to community agencies, and report suspected cases of child & adult abuse/neglect/exploitation. This role also serves as a liaison between the hospital, patients, and the Department of Social Services and responds to requests for consultative services after normal working hours.
