Are you a compassionate Clinical Social Worker looking to make a meaningful impact in primary care? We are seeking a full-time, 40-hr Social Worker to join our Family Medicine team! This is a hybrid position, with four onsite days located at 100 Cummings Center in Beverly, MA and one remote day. Become a vital member of our ambulatory care teams, helping patients navigate complex psychosocial challenges while advocating for their well-being. This hybrid role offers the opportunity to work both in-clinic and remotely, providing flexibility while ensuring comprehensive patient support.

Requirements

  • Master’s degree in Social Work, Mental Health, or Behavioral Medicine
  • Valid Class D Passenger Vehicle Driver’s License

Nice To Haves

  • At least 2 years of clinical experience in a medical setting
  • Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW)
  • Licensed Mental Health Counselor (LMHC) considered

Responsibilities

  • Performs social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.
  • Performs comprehensive assessments and develops treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations.
  • Works collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
  • Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
  • Formulates biopsychosocial assessment, disposition, and treatment plans.
  • Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
  • Monitors, evaluates, and records client progress according to measurable goals described in the treatment and care plan.
