Seeking a master’s‑prepared social worker with psychiatric experience to deliver comprehensive medical social work services in a primary care office. This role involves interviewing patients and their social support systems to complete psychosocial evaluations, providing counseling and support, and assisting patients in coping with stress related to illness and hospitalization. The social worker will collaborate with a multidisciplinary team to develop and implement care plans, serve as a resource to staff and physicians on psychosocial aspects of patient care, and intervene in crisis situations. Responsibilities also include completing psychosocial assessments for post-hospital care, working with the Clinical Care Management team to determine appropriate care settings, and participating in multidisciplinary care planning meetings. The role requires maintaining knowledge of community resources, making referrals, reporting suspected abuse/neglect/exploitation, and serving as a liaison with the Department of Social Services. Additionally, the social worker will identify and address barriers to healthcare transitions and contribute to system improvements.
