About The Position

DaVita is hiring an Intermediate Social Worker to serve as a vital member of each patient's core care team in an outpatient dialysis setting. The Social Worker supports and advocates for patients as they adjust to life on dialysis, helps them understand their rights and responsibilities, and guides them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. The role involves building meaningful, long-term relationships with patients and families and collaborating in a positive, supportive team environment. Requirements

• Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
• Meet all state-required regulations to practice in the dialysis setting.
• Hold a clinical license (e.g., LCSW, LICSW) and meet all state licensure requirements.
• Be proficient in all Experienced-level Social Worker duties.
• Have intermediate computer skills.
• Demonstrate knowledge of government and private insurance programs.
• Have excellent interpersonal and communication skills.
