About The Position
The licensed social worker assists patients in reaching their highest level of physical, mental, and psychosocial well-being and quality of life. The position is in a large transitional care, long-term care, and memory care unit at the Minnesota Masonic Home. New graduates and experienced candidates are welcome.
Requirements
- Bachelor's degree in Social Work.
- Current Minnesota social work licensure.
- Excellent documentation and assessment skills.
- Ability to work independently and as a team member.
Responsibilities
- Facilitate admissions.
- Conduct assessments.
- Advocate for patients and families.
- Coordinate care conferences.
- Complete documentation.
- Coordinate services.
- Plan discharges.
- Perform other duties as assigned.
