About The Position

The Corewell Health Trauma Recovery Center (TRC) provides mental health treatment, psycho-social support, and connectivity to resources for survivors of violent crime and families of homicide victims. While Grand Rapids and West Michigan have community, medical and psychological resources available to survivors of trauma, Corewell Health’s Trauma Recovery Center is the only clinic providing comprehensive, wrap-around care to address all the individual’s needs. The TRC model was created by the University of California-San Francisco (UCSF) to increase access to trauma-informed quality care for all people impacted by violence and systemic inequities. The Social Worker position will be providing case management services to patients in the TRC program. This could include helping patients submit victim compensation applications, housing assistance, applying for insurance, transportation assistance, and linking clients to community resources. Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Requirements

  • Bachelor's Degree Social Work, Exceptions for persons without a social work degree can be made for those who have a human services degree and carry a current social work license
  • 1 year of relevant experience
  • Supervised social work experience in health care setting working directly with individuals. (Rehab and Nursing Centers only)
  • LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan Upon Hire

Nice To Haves

  • 1 year of relevant experience skilled nursing home process and procedure
  • 1 year of relevant experience mental health, substance abuse assessment and treatment
  • 1 year of relevant experience community resources and discharge planning
  • 3 years of relevant experience Hospital or Nursing Home Social work

Responsibilities

  • Responsible for, and will participate in, the assessment, planning, implementing, coordinating, communicating and finalizing the discharge of the patient/resident from the hospital setting or rehab and nursing center.
  • Works with other members of the health care team both inpatient and outpatient, to advocate for and ensure appropriate aftercare and follow up.
