The Social Worker is part of HOPE (Housing Outreach Preventing Eviction), a team that supports clients in Vancouver to maintain their housing. HOPE focuses on offering a short-term intervention based on intensive deep-cleaning and/or household management supports that address immediate risks of eviction, life safety issues, or barriers to discharge from hospital. The HOPE Social Worker plays a key role in overseeing the intake, assessment, intervention, and discharge planning for all HOPE clients. Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Social Worker is responsible for providing clinical and consultative social work services to clients and families. Duties include conducting psychosocial assessments, providing counselling services, maintaining records, attending interdisciplinary clinical/case conferences, identifying applicable community resources for clients and facilitating care throughout the continuum.
