About The Position

DaVita is hiring a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The Social Worker supports and advocates for patients as they adjust to life on dialysis and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. The role also helps patients understand their rights and responsibilities and builds meaningful, long-term relationships with patients and their families. The position is part of a collaborative team that values work-life balance and supports personal and professional growth. Requirements

Master’s Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
Meet all state-required regulations to practice in the dialysis setting.
Hold a clinical license (e.g., LCSW, LICSW) and meet all state licensure requirements.
Be proficient in all Experienced-level Social Worker duties.
Have intermediate computer skills.
Demonstrate knowledge of government and private insurance programs.
Have excellent interpersonal and communication skills.

Responsibilities

Support and advocate for patients transitioning to life on dialysis.
Help patients understand their rights and responsibilities.
Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
Build meaningful, long-term relationships with patients and their families in an outpatient setting.
