It involves building long-term relationships with patients and families and advocating for patients during the transition to life on dialysis. The schedule is 32–40 hours per week, Monday through Friday, generally between 8:00 a.m. and 4:30 p.m., with some flexibility based on patient needs. The caseload is typically around 100–125 patients distributed across the three clinics. The position requires working independently while drawing on support from the social work team. Travel between assigned clinics is required; social worker meetings are generally held no more than once per quarter.
