About The Position

DaVita is hiring a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The Social Worker supports and advocates for patients adjusting to life on dialysis, helping them understand their rights and responsibilities and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. The role emphasizes building meaningful, long-term relationships with patients and their families, and working within a supportive team environment. Paid training is provided.

Requirements

• Master’s degree in Social Work (MSW), with a specialization in clinical practice.
• Active Associate Clinical Social Worker (ASW) status.
• Registration with the California Board of Behavioral Sciences (BBS).
• Meet all state-required regulations to practice in the dialysis setting.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.

Responsibilities

• Support and advocate for patients adjusting to life on dialysis.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Build meaningful, long-term relationships with patients and their families.
• Participate in a patient-centered care team and contribute to patient health through goal-setting and quality improvement initiatives.
