About The Position
Hospital Sisters Health System is seeking a Social Worker to coordinate and provide social services for patients of all ages in their care setting. The role supports patients and families as they navigate the medical system, assesses and monitors their mental and emotional health, provides short-term counseling and therapy, and communicates patient needs and concerns to the broader medical team. This position is in Care Management and has a Behavioral Health/Social Work core function. It is PRN and on-call, with a hybrid work arrangement, at St. Vincent's Hospital in Green Bay, Wisconsin. The listed pay range is $29.55–$44.32; actual pay depends on relevant experience, skills, training, certifications, and education. The position has zero scheduled weekly hours. For remote work, minimum internet speeds are 20 Mbps download, 6 Mbps upload, and latency under 100 ms; higher speeds are preferred.
Requirements
- A master's degree in social work is required. A bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
- A Certified Social Worker (CSW) license in Wisconsin or Licensed Social Worker (LSW) license in Illinois is required.
- An eligible temporary Social Worker license while completing examination requirements for full licensure is required; full licensure must be obtained within the timeframe permitted under the temporary license and applicable state regulations.
- Basic Life Support (BLS) certification is required.
- If travel is required for the role, a valid driver's license, automobile insurance, and personal transportation are required.
- For remote work, internet must provide at least 20 Mbps download, 6 Mbps upload, and latency under 100 ms.
Nice To Haves
- Prior social work or social service clinical experience directly transferable to a healthcare environment is preferred.
- Two years of home care, hospice, discharge planning, or related experience is preferred.
- In Wisconsin, a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) license is preferred.
- In Illinois, a Licensed Clinical Social Worker (LCSW) license is preferred.
- Preferred internet speeds are 100 Mbps or higher download, 10 Mbps or higher upload, and latency under 60 ms.
