About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Social Worker supports hospice patients and families by assessing psychosocial, grief, financial, environmental, and community needs; providing counseling and crisis intervention; and connecting families with community resources. The role also involves completing documentation, participating in assigned on-call service, and ensuring compliance with applicable laws, regulations, and agency policies.

Requirements

Bachelor's or master's degree in social work from a state-accredited program.
Current state license as required by state regulations.
Current driver's license, reliable transportation, and ongoing valid and current auto liability insurance.
One year of experience as a Social Worker in a clinical care setting.

Nice To Haves

Hospice experience preferred.

Responsibilities

Assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs, as reflected in plans of care, clinical records, team meeting minutes, and community resource referrals.
Provide individual patient and family counseling and crisis intervention.
Provide information and assistance with advance directives, funeral planning, and transfers of responsibilities related to legal and fiscal issues.
Facilitate access to community resources based on patient and family needs.
Accept clinical assignments consistent with education and competence to meet patient needs.
Perform on-call responsibilities and provide on-call service to patients and families as assigned.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
