Requirements
- Certified Social Worker (CSW) in Nebraska upon hire
- Minimum of a bachelor's degree in Social Work
- Basic Life Support (BLS) from the American Heart Association upon hire

Responsibilities
- Proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team.
- Champion patient goals, considering biophysical, psychosocial, environmental, financial, and discharge planning needs.
- Collaborate with care partners to craft appropriate discharge plans and secure timely referrals.
- Serve as a vital advocate for patients and families, procuring essential services and acting as a key liaison to post-hospital care and community health resources.
- Educate patients, families, and medical staff on available support, providing application assistance for crucial resources.
