About The Position
DaVita is seeking a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The Social Worker supports and advocates for patients as they adjust to life on dialysis, helping them understand their rights and responsibilities and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. The role focuses on building meaningful, long-term relationships with patients and families and collaborating with a supportive care team. It also involves supporting patient health through clinical goal-setting and quality improvement initiatives.
Requirements
- Meet all state-required regulations to practice in the dialysis setting.
- Master's degree in Social Work (MSW) with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Functional proficiency with DaVita-specific applications within 60 days.
Responsibilities
- Support and advocate for patients as they transition to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
- Build meaningful, long-term relationships with patients and their families in an outpatient setting.
- Contribute to clinical goal-setting and quality improvement initiatives aimed at improving patient health.
