About The Position

Catholic Eldercare is seeking an organized, person-centered Social Worker to join our team in coordinating services and meeting the psycho-social needs of our residents and their representatives. Catholic Eldercare offers compassionate, person-centered care, housing, and services to all seniors across the continuum of care. For over 40 years, Catholic Eldercare has been a vital part of the NE Minneapolis community meeting the needs of elders in a caring, dignified atmosphere. Requirements

  • Degree in Social Work or related
  • Current MN Licensure in Social Work or ability to successfully obtain license within 60 days.
  • Proficiency in Microsoft Office and digital literacy for charting and interfaces with outside services.
  • Ability to pass NetStudy 2.0 background screening.
  • Effective Communicator and active listener who seeks to understand - in person, via electronic media, and in writing.
  • Social sensitivity and awareness of the values, practices, and reactions of others.
  • Service Orientation with a strong desire to help others and assumes their positive intent.
  • Positive, person-centered approach with resilience in dealing with difficult situations.
  • Demonstrated ability to effectively manage time and work priorities.
  • Organized, self-driven individual who functions well as part of a team.

Responsibilities

  • Completing admission and discharge process and paperwork for facility residents.
  • Identifying needed social services and assisting with the provision of these services
  • Monitoring the psycho-social and emotional needs of residents
  • Creating care plans and making amendments as needed
  • Balancing resident caseload and charting.
  • Providing information and referral services as needed.
  • Coordinating all parties in resident care conferences/special meetings with the multi-disciplinary team.
  • Completing assigned MDS interviews and MDS social services assessments.
  • Assisting the resident and family with room changes.
  • Maintaining a proactive understanding of public and private services related to the financial and psycho-social needs of residents (health insurance Medical Assistance, PAS, ACG programs, etc.).
  • Overseeing Medicaid- and Medicare-related paperwork and recordkeeping.
  • Responding to resident/family/staff concerns using confidentiality and judgment
  • Advocating for residents’ rights.
  • Coordinate and update potential discharge plans for each resident.
  • Improving skills through training and meetings to stay informed on changing regulations and current best practices.
  • Lead/coordinate TCU quality committee, and complete guest satisfaction surveys.
  • Following facility standards for infection control and safety.
  • Working as a member of the Social Work team to meet resident and/or staffing needs.
  • Educate clients about end-of-life symptoms and options to help them make informed decisions.
  • Performing other duties as assigned.
