About The Position

The Oxford Rehabilitation & Health Care Center is hiring a part-time Social Worker for its 120-bed skilled nursing facility, which serves a large SUDS population. The role plans, organizes, develops, and directs required social services to address residents’ medically related emotional and social needs on an individual basis. The shift is 8 a.m.–4 p.m., with flexible days, and pay is $35–$38 per hour, paid weekly.

Requirements

Must possess a Massachusetts social work license (LSW, LICSW, LSWA, and/or LCSW).
Must possess at least a bachelor’s degree in social work from an approved school of social work.
Staff must be up to date with COVID and flu vaccinations or acknowledge a declination of the vaccination(s), in accordance with facility policy and federal and state guidelines.

Nice To Haves

At least one year of social work experience in a hospital, skilled facility, or other health-related field is preferred.

Responsibilities

Coordinate and monitor resident care conferences.
Work with residents, responsible parties, and families to support adjustment to placement and to crises, including illness, disability, and death.
Comply with facility privacy policies and procedures and protect residents’ individual health information.
Complete comprehensive documentation in a timely manner.
Advocate for residents and coordinate their personal needs.
Use community resources appropriately.
Support staff with resident-related issues.
Perform other appropriate duties assigned by the supervisor.
