About The Position

The incumbent will serve as an Outpatient/Inpatient Social Worker and is responsible for providing direct supervision and support for a designated group of high-risk patients. The primary focus is to design and manage a continuum of care that empowers clients toward achieving demonstrable outcomes and self-sufficiency.

Requirements

  • Master’s degree in social work (MSW).
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with a valid, unrestricted license to practice in any U.S. state, territory, or the Commonwealth of Puerto Rico.
  • Current Basic Life Support (BLS) certification by the American Heart Association is required.
  • Minimum of three years in a clinical care setting, with knowledge and experience in discharge planning, case management, and utilization review processes.
  • Demonstrated proficiency in using electronic health record systems.

Responsibilities

  • Assess client needs and develop comprehensive service plans.
  • Acquire necessary resources and monitor client progress while responding to emerging needs.
  • Facilitate connections between clients and community social service providers, healthcare providers, and mental health specialists.
  • Utilize patient case reviews to ensure medical necessity of referrals to specialty providers for services outside of facility capabilities.
  • Collaborate with a multidisciplinary team to promote effective use of healthcare resources and ensure a seamless continuum of care.
  • Evaluate patients in the emergency department for care coordination needs and potential admissions.
  • Propose outpatient plans for safe discharge with physician approval.
  • Coordinate care for patients facing issues related to abuse, chronic pain, and frequent utilization.
  • Interact constructively with patients across all age groups, from newborns to geriatric populations, including those with mental health concerns.
