About The Position
The Hospice teams support patients and family members through the final stages of their illness. Services are offered in various settings, including the patient's residence, skilled nursing facilities, assisted living facilities, and adult foster homes. These include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, hospice volunteers, and bereavement support.
Requirements
- Oregon LMSW or CSWA licensure required.
- Healthcare Provider Level BLS required.
- Use of employee's personal automobile.
- Valid driver's license.
- Auto liability/property damage insurance.
- Vehicle must be maintained in good working order.
Nice To Haves
- Oregon LCSW preferred.
- Experience as a social worker in a hospital or medical setting preferred.
Responsibilities
- Provide medical social services and counseling to patients and their families.
- Offer spiritual support.
- Coordinate with hospice aides and volunteers.
- Provide bereavement support.
