The role of the Social Worker is to create an environment in the home that provides opportunity for the community members to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners, which will enhance the quality of life for residents and will promote the health of the intentional community of home. The Social Worker will know each resident and their family, including a psychosocial profile of the resident and an understanding of the family members and dynamics. They will participate in care planning for resident needs, assure that out-of-facility activities are available to residents, and understand and complete MDSs timely and accurately. This role also involves leading the Grievance Committee, hearing resident concerns, and following up on them timely. The Social Worker will understand the ability to complete investigations with the assistance of the Executive Director, provide annual education on Abuse, Neglect, and Misappropriation, and issue NOMNC and ABN information, allowing for the appeal process. They will support and encourage social work interns as needed and include families in the planning process. Understanding the process for Medicaid Waiver and helping support transitions of care is also a key responsibility. The Social Worker will evaluate the effectiveness of programs and activities, provide weekly support to each level of care (Independent Living, Assisted Living, and Nursing), and complete PAS/RR and LOCs on admission. Documentation will be in accordance with the organization’s policies. The role also includes completing admission assessments, coordinating resident discharges, coordinating care conferences, and performing any other duties as assigned.
