Requirements
- Master’s Degree from a school of Social work accredited by the Council of Social Work Educations
- 1 year social work experience in a health care setting
- CPR - Cardiac Pulmonary Resuscitation Upon Hire
- DL NUMBER - Driver License, Valid and in State Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation Upon Hire
Responsibilities
- Assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation.
- Assists patients with community resource planning for identified long term care needs.
- Assists patients with obtaining information regarding advance directives.
- Updates case records.
