About The Position

Kahl Home, a faith-based, nonprofit short-term rehabilitation and skilled geriatric care facility sponsored by the Carmelite Sisters for the Aged and Infirm since 1955, is seeking a Social Worker. The role supports residents and their families through admission, assessment, care planning, and discharge processes, and includes performing Admission Coordinator duties when the coordinator is unavailable. Requirements

At least 3 years of long-term care experience.
Iowa Social Worker License or a Master's Degree in Social Work.
Knowledge of MDS 3.0 and Point Click Care.
Knowledge of electronic medical records (EMRs).
Fluent in reading, writing, and speaking English.

Responsibilities

Assist with documentation needed for resident admission to the facility.
Evaluate and address residents’ needs and concerns.
Facilitate patient and family discharge to the next level of care.
Complete required documentation in Point Click Care (PCC).
Interview residents and families to assess residents’ psychosocial strengths and weaknesses.
Update resident files and charts with discharge plans, progress notes, plans of care, and assessments.
Participate in interdisciplinary care conference meetings to assess and review residents’ progress.
Assist residents and/or family members with emotional, financial, and other needs.
Communicate with families and notify them of new or apparent needs or changes in level of care.
Perform Admission Coordinator duties when the coordinator is unavailable.
