Social Worker - Case Management
About The Position
The Social Worker creates optional outcomes for the patient and family by managing complex psychosocial, medical, and economic co-morbidities. Through advanced practice skills, they mobilize resources to reduce risk and function as an ambassador between the Hospital and community. The Social Worker is responsible for providing high-quality patient care, displaying good skills in assessment, treatment planning, individual and family intervention, and discharge planning. The Social Worker functions well as a member of the multidisciplinary team.
