About The Position

The Social Worker creates optional outcomes for the patient and family by managing complex psychosocial, medical, and economic co-morbidities. Through advanced practice skills mobilizes resources to reduce risk and functions as ambassador between Hospital and community. The Social Worker is responsible for providing high quality patient care, displaying good skills in assessment, treatment planning, individual and family intervention, and discharge planning. The Social Worker functions well as a member of the multidisciplinary team.

Requirements

• Bachelors or Masters Prepared Social Worker; Bachelors or Masters prepared Rehab Counselor, or Bachelors or Masters Prepared Mental Health Counselor
• One (1) year experience in social work services which includes care planning and referrals to community agencies, community/public setting or medical social work necessary
• American Heart Association BLS

• LCSW is only required for student supervision
