The Social Worker-LCSW creates optimal outcomes for patients and families by managing complex psychosocial and economic comorbidities. Using advanced practice skills, the social worker mobilizes resources to reduce risk and serves as a liaison between the hospital and community. The role provides social work services that help patients and families benefit from treatment, cope with their medical conditions, and participate in care. It also coordinates transitions of care for patients with high psychosocial needs and maintains current knowledge of local, state, and federal services that can provide education and resources. The social worker supports HIM and Patient Accounts by ensuring timely, accurate, and complete data entry in multiple information systems and databases. In collaboration with the Care Coordinator and interdisciplinary team, the role reviews the multidisciplinary plan of care in the patient chart, including estimated length of stay, tentative discharge date, assessed discharge needs, and the plan discussed with the patient or family. The social worker attends multidisciplinary rounds or huddles, reports on psychosocial status and its implications for transitions of care, monitors progress toward social work plan-of-care goals, and revises the plan as patient needs or condition change. The role assesses potential barriers to successful community reintegration, including clinical, social, support, and home-environment factors, and escalates concerns through appropriate channels for resolution. Job duties are performed in accordance with Tampa General Hospital’s mission, vision, and values. This is a contract opportunity. The employer describes contract assignments as 13–26 weeks, with possible extensions based on business needs and mutual interest. Contract roles are designed with a focus on conversion to permanent employment.
