About The Position

Georgia Hospice Care is seeking passionate and committed hospice medical social workers to join their team. As a hospice medical social worker, you will be responsible for psychosocial evaluations, ongoing counsel, bereavement services, and community education outreach. You will provide physical, emotional, and spiritual support to patients and families in accordance with the plan of care. This role offers access to supportive leadership and professional development opportunities with room for advancement.

Requirements

A heart to serve patients and families and a passion for providing the best possible care.
MS degree in social work from an accredited school of social work approved by the Council of Social Work Education.
Current state license as a social worker.
2+ years of clinical work experience, preferably in healthcare or hospice.
Reliable transportation.
Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively.

Responsibilities

Perform psychosocial evaluations.
Provide ongoing counsel to patients and families.
Offer bereavement services.
Conduct community education outreach.
Provide physical, emotional, and spiritual support to patients and families in accordance with the plan of care.
