The social worker, either contracted or employed by the organization, is responsible for implementing standards of care for medical social work services and ensuring adherence to all terms of the Service or Employment Agreement. This role involves assessing the psychosocial status of patients and their families/caregivers, conducting social evaluations, providing counseling and crisis intervention, maintaining clinical records, educating on advanced directives, and offering information and referral services. The social worker acts as a liaison between patients, families, and community agencies, collaborates with the interdisciplinary group to integrate psychosocial factors into patient care plans, and assists with planning for end-of-life arrangements. The position requires active participation in quality assessment and performance improvement activities.
