About The Position

Amedisys is hiring a full-time, first-shift medical social worker for hospice care. The role provides social work services in accordance with each patient's plan of care and helps patients and families address health-related financial, social, and emotional concerns and develop effective coping skills. The social worker educates patients, families, and care team members about medical social work interventions and the treatment plan; supports care planning, transfers, and discharges; and identifies and addresses risks in the patient's environment in coordination with the physician and healthcare team. The position requires a master's or doctorate degree from a Council on Social Work Education-accredited school of social work, one year of social work experience in a healthcare setting, applicable state and federal qualifications, and current CPR certification. Compensation is $79,000–$90,000, varying by geographic market and job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift; participation in some benefits requires enrollment in an Amedisys medical plan.

Requirements

• Master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a healthcare setting.
• Meet state and federal requirements for the position.
• Current CPR certification.

Responsibilities

• Assist patients and families with health-related financial, social, and emotional concerns, including developing effective coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in a patient's clinical status promptly to other healthcare team members and care center office staff.
• Educate patients, family members, and other team members about medical social work interventions and the treatment plan.
• Perform care planning, transfer, and discharge functions.
• Identify risks or threats to a patient's health in their environment and promptly implement appropriate interventions to reduce them.
• Communicate identified risks or threats to the physician and other healthcare team members to coordinate care effectively.
• Perform other duties as assigned.
