About The Position
Provides ongoing psychosocial assessment, treatment, and discharge planning for a variety of patients in a medical setting. Communicates clearly across the continuum of care to improve patient outcomes and manages an assigned caseload of patients.
Requirements
- Master’s degree in Social Work.
- Licensure as a Social Worker by the State of New Jersey.
