About The Position
Juniper Communities is seeking a compassionate and caring Social Worker for the Spearly Center. This full-time position involves working Tuesday to Saturday with rotational on-call duties. The role is within a skilled nursing facility specializing in mental and behavioral health, catering to residents with conditions such as schizophrenia, bipolar disorder, TBI, failed suicide attempts, and more, in addition to typical Alzheimer's/dementia diagnoses. The ideal candidate will care for and value residents in need.
Requirements
- 1 year of experience.
- Must pass a background and drug test.

- BSW in Social Work or related field.
Responsibilities
- Provide social work services to residents in a skilled nursing facility specializing in mental and behavioral health.
- Assist residents with various needs related to their diagnoses, including schizophrenia, bipolar disorder, TBI, and failed suicide attempts.
- Manage knowledge of Medicaid/Medicare, VA, hospice, ULTC's, Social Security, Title 19, MCD redetermination and application process, ancillary scheduling and facilitation, and representative payee services.
