About The Position

UChicago Medicine is seeking an Adult Inpatient Social Worker for its Social Work department. This registry position provides support and coverage as needed in a fast-paced hospital to facilitate timely and safe patient discharges. The social worker joins a multidisciplinary team providing consultation, assessment, and interventions for patients and families who need emotional, social, or psychological support.

Requirements

Master’s degree in social work from a School of Social Work accredited by the Council on Social Work Education.
Current Illinois Social Work License; LSW required.
Knowledge of principles of growth and development across the lifespan.

Nice To Haves

Licensed Clinical Social Worker (LCSW).
At least one year of clinical social work experience in an acute care hospital setting.

Responsibilities

Develops and leads or co-leads support and psychoeducational groups for designated patient and family populations, as appropriate.
Collaboratively formulates appropriate and realistic discharge plans with the interdisciplinary healthcare team and community-based organizations, including the Wyler Child Protective Services team and DCFS.
Performs case management functions, including information and referral services and connections to appropriate hospital resources and community-based organizations.
Serves as a consultant and educator to care and multidisciplinary teams regarding biopsychosocial aspects of illness and hospitalization.
Attends and participates in multidisciplinary patient care rounds as available.
Provides administrative coverage when requested.
Participates in the social work data collection process.
Develops accurate psychosocial assessments addressing current problems and relevant biopsychosocial and environmental factors.
