About The Position

NHC is hiring a full-time Social Worker at NHC HealthCare Bluffton in Okatie, South Carolina. The Social Worker advocates for patients and their families, supports adjustment to center life and to crises involving illness, disability, and death, and coordinates discharge planning. The role works with professionals from other disciplines to develop patient care plans and reports the status and needs of social services to the Director of Social Work Services. Requirements

Bachelor’s or master’s degree in social work from an accredited program; a bachelor’s degree is listed as preferred in the experience section.
Knowledge of the medical case management process and the needs of geriatric patients.
Knowledge of healthcare settings and funding resources, including Medicare Parts A, B, C, and D, Medicaid, managed care, and other private insurance.
Imagination and creative ability.
Ability to work cooperatively with other professionals.
Ability to organize and carry out responsibilities efficiently and effectively.

Responsibilities
