About The Position

DaVita is seeking a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The role supports and advocates for patients navigating the transition to dialysis and the physical, mental, emotional, and financial demands of End Stage Renal Disease. The Social Worker helps patients understand their rights and responsibilities and builds meaningful, long-term relationships with patients and their families.

Requirements

• Meet all state-required regulations to practice in the dialysis setting.
• Master’s degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.

Responsibilities

• Support and advocate for patients receiving dialysis care.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Build meaningful, long-term relationships with patients and their families.
• Contribute to a positive team environment and participate in patient-health improvement through clinical goal-setting and quality improvement initiatives.
