Social Worker - Ambulatory Care Network - Days Our Audubon Practice which is part of our Ambulatory Care Network provides services such as triage, screening, referral, and crisis intervention to ensure the mental well-being of our patients. You will be responsible for seeing patients virtually and in person, via scheduled appointments and walk ins.
Requirements
- Master's Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW)
Nice To Haves
- Bilingual Spanish
- Experience in a busy outpatient setting
- Experience working with interdisciplinary teams
Responsibilities
- Crisis intervention
- Assessing for safety using evidence based screening
- Concrete services
- Assisting and following up with home care
- Psychosocial assessments
- Referrals to CBO's
- Work collaboratively with interdisciplinary team and across the network of ambulatory care clinics
