Requirements

• Bachelors or Master’s degree in Social Work from an accredited program.
• Current Social Work license through the Ohio Counselor, Social Worker, and Marriage & Family Therapist Board.
• CPR certification (BLS - Basic Life Support).
• At least 1 year of professional social work experience in a healthcare or related setting.
• Ability to handle confidential information and make thoughtful, independent decisions in a fast-paced environment.

Responsibilities

• Provide direct care, counseling, and crisis intervention to patients and their families across all age groups.
• Support patients during discharge planning, ensuring a smooth transition of care and access to community resources.
• Serve as a key member of a multidisciplinary care team, contributing to treatment planning and decision-making.
• Coordinate referrals to appropriate internal and external support services.
• Promote and participate in patient and staff educational programs.
