About The Position

The Social Worker will evaluate potential clients as a total person, identifying functional limitations that impede independent living. This role involves conducting in-depth assessments, developing care plans, implementing services, and monitoring client progress. The Social Worker will also identify and develop support systems for clients, consult with healthcare practitioners, and maintain case records and reports. A key aspect of the role is providing case management in accordance with departmental policy, program standards, and patient needs, including health and psychosocial status evaluations, care plan development, and crisis intervention.

Requirements

  • Bachelor’s degree required in social work, psychology, counseling, rehabilitation, gerontology or sociology.
  • Two years of experience required working with the elderly.
  • Two years of case management experience required.
  • Clinical assessment skills.
  • Knowledge of the community.
  • Ability to work with multidisciplinary team.
  • Ability to work with clients/families in the home setting.
  • Knowledge of aging and programs for seniors.

Responsibilities

  • Evaluate the potential client as a total person and identify the functional limitations, which impede independent living.
  • Conduct in-depth assessments and reassessments covering psychosocial, rehabilitation, and environmental concerns.
  • Refer clients to special consultants to provide detailed evaluations and recommendations to improve the client’s functional level.
  • Develop the care plan, implement the services and conduct follow up and monitoring of the plan.
  • Identify and develop support systems for the client.
  • Consult with the Health Practitioner.
  • Case recording and reporting.
  • Provide case management in accordance with departmental policy, program standards and patient needs.
  • Complete assessment evaluation of a client’s health and psychosocial status utilizing all relevant information sources including the identification of issues that are specific to age, diagnosis and disabilities of the client population served, in accordance with departmental policy.
  • Participate as an active team member in care planning to develop care plans specific to the client’s needs.
  • Implement interventions and services utilizing community resources in a timely fashion in accordance with departmental policy.
  • Maintain and document regular contact with clients in accordance with departmental policy.
  • Identify, assess and respond to crisis situations in a timely fashion, with appropriate interventions.
  • Observe all legal, departmental, and/or hospital regulations.
