About The Position

The Social Service Worker at the UHN Reactivation Care Centre provides psychosocial assessment, care planning, advocacy, and discharge support for patients requiring rehabilitation and restorative care. Working collaboratively within an interdisciplinary team, the Social Service Worker helps patients and families navigate healthcare and community resources, addresses social determinants of health, and supports safe, patient-centered transitions to home or alternative care settings. The role promotes patient independence, well-being, and quality of life through counseling, crisis intervention, resource coordination, and family support while ensuring care aligns with patient goals and organizational standards.

Requirements

  • Completion of a Masters Degree in Social Work (M.S.W.), or recognized equivalent required.
  • Membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required.
  • Excellent interpersonal skills required.
  • Excellent verbal and written communication skills required.
  • Excellent organizational and time management skills required.
  • Excellent problem solving skills required.
  • Knowledge of policies, legislation, programs and issues related to social welfare and health care institutions and services required.

Nice To Haves

  • Two (2) years experience working with a multi-disciplinary team in a health care setting preferred.
  • Experience working with program specific patients and families an asset.
  • Research experience/education an asset.
  • Student education/supervision an asset.
  • Computer knowledge preferred, i.e., Patient Care System and Microsoft Office.
  • Knowledge of a second language, an asset.

Responsibilities

  • Assessing patients for the purpose of evaluating the need and appropriateness for service, the nature of problem and the formulation of recommendations or treatment plans
  • Implementing an intervention plan with patients, family members, significant others and the multi-disciplinary team for the purpose of facilitating changes in behaviour, attitudes, feelings and/or environment
  • Reassessing and modifying the intervention plan, as required
  • Liaising with appropriate community agencies
  • Facilitating timely transfers of patients to appropriate alternate level of care facilities
  • Establishing contact with members of the multi-disciplinary team and community agencies to obtain, provide or exchange information relating to the patient’s psycho-social functioning and health care in the past, present and future
  • Rendering services for the immediate benefit of the Hospital and the community
  • Providing clinical teaching to graduate social work students, medical students and professional colleagues at University Health Network and at Faculties of Social Work and in the Community
  • Developing and participating in patient education programs
  • Participating in program planning and evaluation
  • Participating in research activities, projects
  • Performing activities required for the effective and efficient operation/maintenance of Social Work practice
  • Performing other duties consistent with the job classification, as required.
