About The Position
DaVita is hiring a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The role supports and advocates for patients as they adjust to life on dialysis and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. The Social Worker helps patients understand their rights and responsibilities and builds meaningful, long-term relationships with patients and their families. The position is part of a supportive team that values work-life balance and professional growth.
Requirements
- Meet all state-required regulations to practice in the dialysis setting.
- Master’s degree in Social Work (MSW) with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Achieve functional proficiency with DaVita-specific applications within 60 days.
Responsibilities
- Support and advocate for patients as they adjust to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
- Build meaningful, long-term relationships with patients and their families in an outpatient setting.
- Participate in a team environment that supports patient care and quality improvement.
