About The Position
Landmark Hospital of Joplin is looking for a compassionate, detail-driven Social Worker to join our interdisciplinary care team. If you thrive on helping patients navigate complex transitions and want to work in a setting where every case gets real attention, we'd love to talk with you. As our Social Worker, you'll partner with physicians, nurses, and case managers to plan safe, timely discharges for medically complex patients. You'll be a key advocate for patients and families — helping them understand their options, connect with the right resources, and move confidently into the next stage of care.
Requirements
- Bachelor's degree in Social Work required
- 5+ years of hospital-based social work experience
- Strong working knowledge of community resources, referral agencies, and healthcare systems
- Sharp analytical and critical-thinking skills, with the ability to manage multiple complex cases at once
- A collaborative, empathetic approach and excellent communication skills across a multidisciplinary team
Nice To Haves
- Master's degree preferred
- Active, unrestricted Missouri LMSW or LCSW preferred
Responsibilities
- Support patients and families through discharge planning, financial concerns, and the emotional impact of acute illness
- Provide supportive counseling as patients adjust to illness and address broader biopsychosocial needs
- Coordinate with external agencies to ensure a smooth, well-informed transition out of the hospital
