About The Position

Family Service League, established in 1926, is a social services agency dedicated to transforming lives and communities through mental health, addiction, housing, and essential human services, operating over 60 social service programs across more than 20 locations. The organization is seeking a full-time, Master’s-level Social Worker for its School Based Program in Huntington, NY. This role involves providing individual, family, and group therapy for adolescents with a full range of DSM diagnoses within an educational setting, connected to an outpatient mental health clinic. The position is year-round and does not follow a school calendar.

Requirements

  • Master’s degree in relevant human service field required.
  • LMSW, LMHC, LCSW or related licensed clinical degree or limited permit required.
  • At least one year of experience working in a behavioral health setting required.
  • Proficient computer skills, including Microsoft Office and Electronic Health Records required.
  • Excellent interpersonal, and verbal and written communication skills required.
  • Must possess the ability to multitask, problem solve, make clear decisions, and work individually and as part of a team.

Nice To Haves

  • Experience providing school-based mental health counseling is strongly preferred.
  • Bilingual in Spanish preferred, but not required.

Responsibilities

  • Provide individual and group therapy to children/ adolescents with full range of DSM diagnoses in a school-based setting.
  • Direct service will be provided both in person and via telehealth.
  • Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record, including progress notes, comprehensive assessments and Individual Actions Plans.
  • Maintain ongoing communication and collaboration with school personnel, parents/guardians, and community providers.
  • Provide collateral contacts with all relevant referral sources, including family members.
  • Coordinate linkage to community resources and discharge planning.
  • Attend routine supervision/team meetings.
  • Understand and demonstrate a commitment to Performance Quality Insurance (PQI) and accreditation with the Council on Accreditation for Children and Family Services (COA).
  • Provide additional support/services to the clinic as needed.
  • All other duties as assigned.
  • When school is not in session, or as needed during the year, provide services in the outpatient clinic.
  • Conducting intake and assessment appointments.
  • Providing family therapy sessions.
  • Completing reassessments and treatment plan updates.
  • Assisting with continuity of care for students and families.
  • Must be flexible and able to work in both school and clinic environments.
