About The Position

ATA Services, Inc. is seeking a full-time Social Worker IV for their client, Hawaii State Hospital. This is a long-term position located at 45-710 Kea’ahala Road, Kaneohe, HI 96744-3597. The primary purpose of this role is to develop psychosocial assessments and treatment plans as part of a multidisciplinary team, focusing on community reintegration. The role emphasizes group/clinical/patient programming over case management.

Requirements

• Possess a bachelor’s or master’s degree in social work from a program accredited by the Council on Social Work Education, or a doctoral degree in social work from an accredited college or university.
• Education background must demonstrate the ability to write clear and comprehensive reports and other documents.
• Education background must demonstrate the ability to read and interpret complex written material.
• Education background must demonstrate the ability to solve complex problems logically and systematically.
• A minimum of one additional year of professional social work experience equivalent to level III, demonstrating ability to perform the most difficult and complex assignments.

Responsibilities

• Conducts diagnostic interviews, observations, and contacts community treatment providers, forensic coordinator, probation officer, entitlement providers, and family members/significant others.
• Performs evaluation of admission materials and obtains data essential for treatment planning, documenting in a psychosocial admission assessment.
• Collaborates with treatment team members to determine if further diagnostic studies or information are needed.
• Participates in developing individual Master Treatment Plans and leads the development of a transition and discharge plan.
• Coordinates the completion of all applications for discharge, including housing, financial, work, and psychosocial outpatient programs.
• Ensures legal encumbrances are adequately addressed and follows up with probation and parole officers.
• Attends court hearings at Circuit, District, and Family courts as needed.
• Provides individual and group psychotherapy, counseling, supportive therapy, training, or other modalities of treatment.
• Establishes and maintains a therapeutic relationship with assigned patients.
• Coordinates inter-agency staff reports for patients considered appropriate for discharge.
• Prepares and maintains statistical forms and reports related to patient admission, changes of legal status, and discharge.
• Plans and coordinates appropriate discharge and aftercare for each patient, documenting throughout the hospital episode and on the Discharge Summary.
• Provides education and counseling to family, caretakers, and/or other significant persons.
• Completes all required documentation according to current guidelines within required time frames.
• Provides in-patient care management documentation, including treatment goals, interventions, patient treatment plans, patient responses, and related medical data.
• Maintains 30-day documentation of post-discharge care received by the patient.
• Gathers data pertinent to establishing a case for court hearing, identifying key witnesses, and determining the patient's mental condition, stability, and post-hospital plans.
• Assists the Attorney General's Office and/or criminal court staff in preparing an individual case for hearing.
• Testifies in court case hearings regarding clarification of issues of appropriate restrictive facilities or as an eyewitness to dangerousness.
