Saint Francis Ministries is seeking a Social Worker to serve as a key communication link between the placing agency/service and the treatment team. This role involves case managing each client’s individual plan of care through coordination of services both within and outside the Program. The Social Worker will act as the primary liaison between the family, placing agency, Saint Francis Ministries staff, court, education personnel, and any other treatment providers. A key responsibility is facilitating essential planning meetings from the beginning to focus on discharge. The role also includes meeting with clients and families at admission, documenting bio-psychosocial histories, developing and documenting individual plans of care, participating in team decisions, facilitating parent/guardian involvement, attending team meetings, gaining personal acquaintance with clients, facilitating group counseling, liaising with families, establishing relationships with community resources, coordinating recommendations, coordinating with other treatment professionals for discharge, preparing discharge summaries, documenting outside contacts, participating in Performance Improvement and Utilization Review meetings, attending chapel and being involved in the spiritual life of the client, administering prescribed medication, being on call for emergency transportation, reporting critical incidents, implementing Saint Francis Ministries policies, reporting unusual incidents, ensuring clients' rights are protected, and following all safety procedures.
