This role focuses on reducing readmissions and ensuring patient safety by overseeing complex discharge plans. The Social Worker will ensure patients have crucial resources at home, such as housing, follow-up care, and medications. They will tackle non-medical issues and address the root causes, referred to as Social Determinants of Health (SDOH), like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment. The role also involves providing emotional support through mental health counseling, crisis intervention, and connecting patients to behavioral health services. Additionally, the Social Worker will serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge.
