The Social Worker is responsible and accountable for assessing the psychological, social, emotional, cultural, and/or financial needs / discharge services that impact high-risk patients’ health and recovery. Has the ability to utilize social services, supportive counseling, and clinical knowledge in the assessment of these needs. Collaborates with the case manager in the development of a discharge plan and addresses assessed needs in a timely manner. Knowledge of pay or source implications on discharge plan, resources, and placement options to maximize a patient’s successful disposition. The Social Worker performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of this facility. The Social Worker displays positive support for the facility’s values and mission.
